Preferred Provider Program for Anchorage and Mat-Su Valley
The Alaska Teamster-Employer Welfare Trust has entered into a contract with providers in Anchorage and Mat-Su to provide facility services to participants at preferred rates to keep costs low for you and the Plan. This Preferred Provider facility program applies to hospital emergency room services, inpatient hospital services, outpatient hospital services, maternity care and surgical services. Overview
If you are within a 75-mile radius of a Preferred Provider facility, you must use one to receive the maximum (80%) reimbursement from the Plan, and for your out-of-pocket costs to count towards your annual limit, unless they do not offer the care you need.
If you receive care from a non-preferred provider within a 75-mile radius of a Preferred Provider facility that offers the care you require, you will 1) be reimbursed less for Covered Expenses, 2) you will pay an extra deductible, additional out of pocket and a penalty or penalty, and 3) your out-of-pocket costs will not count towards your annual limit. Preferred Provider Facility Details
If you choose to use a provider within a 75-mile radius of a Preferred Provider facility for scheduled inpatient or outpatient services provided by those facilities, your co-insurance (the amount you pay out-of-pocket) will not count towards your annual Out-Of-Pocket Limit.
If you go to a hospital within a 75-mile radius of Alaska Regional or Mat-Su Regional Hospital for inpatient services, the reimbursement rate will be 60% of the Preferred Provider hospital’s negotiated rate plus a $1,000 deductible for each scheduled non-preferred provider hospital admission.
If you go to a hospital or outpatient facility within a 75-mile radius of a Preferred Provider facility for outpatient services provided by those facilities, the reimbursement rate will be 60% after a 50% penalty reduction is applied.
Outpatient services include but are not limited to diagnostic laboratory examinations, chemotherapy, diagnostic x-rays, radium or radioactive isotope therapy, out-patient hemodialysis, and radiation therapy.

Get care with no co-pays at Coalition Health Centers. Walk in or schedule an appointment to access treatment for injury and illness, disease prevention, DOT physicals, health promotion, patient education, and chronic disease management.

No deductible or coinsurance applies—just expert care at no cost to you. Surgical procedures include Cardiac, General, Neurological, Orthopedic, Spine, Vascular, and Women’s Health.
