Preferred Provider Facility Program for Anchorage and Mat-Su
If you are within a 75-mile radius of a Preferred Provider facility, you must use one of these providers to receive the maximum (80%) reimbursement from the Plan, and for your out-of-pocket costs to count towards your annual limit, unless they do not offer the care you require.
If you receive care from a non-preferred provider within a 75-mile radius of a Preferred Provider facility that offers the care you require, you will 1) be reimbursed less for Covered Expenses, 2) you will pay an extra deductible, additional out of pocket and a penalty or penalty, and 3) your out-of-pocket costs will not count towards your annual limit.
