Benefit changes impact everyone on your plan!
Make sure the Alaska Teamster-Employer Welfare Trust has updated contact information for the people in your family covered by the plan. Take a few minutes to update your contact information and add your spouse and adult dependents if applicable.
Note: If you need to add a new or change the status of a dependent, you will need to fill out the Status Change/Special Enrollment Form instead.
